Community Health Services, Inc. is seeking a strategic and goal-oriented Care Coordination Supervisor to support our Adult Medicine Department. The Care Coordination Supervisor is responsible for developing, implementing, and managing procedures to ensure appropriate management of care coordination services.

Manage the Adult Medicine team of care coordinators and work collaboratively with the team to ensure all tasks are completed timely.

Collaborate with the clinical care team to manage and follow up with referrals, consults, discharges, and orders for home care as well as diagnostic and radiologic tests.

Participate in quality and process improvement efforts in the department and the organization.

Represent the organization at local, regional, and statewide boards, commissions, councils, workgroups, and committees as well as government and community events related to care coordination.

Create and maintain internal and external relationships.

Have an Associate's or Bachelor's Degree in Nursing

Have valid State of Connecticut licensure as a Registered Nurse (RN)

Have five (5) years of experience as a registered nurse in an acute or outpatient setting

Are experienced in working with socioeconomically diverse, multiethnic populations

Want to leverage your skills in medicine to support the needs of our fast-paced Adult Medicine department

Are creative in process development for care coordination services
