Community Health Services, Inc. is hiring a Medical Case Manager for its Ryan White  Program.  The Medical Case Manager provides coordination of HIV medical care for eligible clients for various treatments and referrals including labs, screenings, medical specialist appointments, mental health, oral health care, and substance abuse treatment. The MCM provides treatment adherence support and referral to HIV+ clients that includes the provision of counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments. The MCM is part of a multidisciplinary medical care team, with a specific role of assisting clients in following their medical treatment plan and assisting in the coordination and follow-up of the client’s medical care. Medical Case Managers support and advocate for health center patients with internal stakeholders and community resources in order to improve quality of health status and enhance quality of life of individuals living with HIV/AIDS. .

The successful candidate will:
  • Have a broad base of knowledge regarding HIV diseases, related health and psychosocial issues and risk reduction strategies for hard-to-reach populations.

  • Experience working with individuals living with HIV, addiction, mental illness, trauma and violence.

  • Possess skill in motivational interviewing and communicating with patients to conduct accurate assessments and engage in treatment planning and crisis intervention. 

  • Demonstrate compassion while counseling clients regarding their entitlement to benefits and while making referrals for assistance with accessing private and public programs such as Medicaid, Medicare and other insurance market exchanges.

  • Have the organization and time management skills to ensure timely and coordinated access to medically appropriate levels of health and supportive services and continuity of care through ongoing assessment of patient needs.

  • Excellent interpersonal verbal and written communication skill and the ability to work cooperatively and collaboratively with interdisciplinary health professionals, other service providers and a diverse and multi-cultural client/patient population.

  • Be able to establish and maintain good working relationships with internal, external and governmental resources.

  • Have demonstrated success advocating for patients to help them obtain culturally and linguistically appropriate services.
