Community Health Services, Inc. is seeking a Healthy Start Coordinator to implement the goals provided by the Healthy Start Program, which includes the reduction of the infant mortality rate and improvement of the health and well-being of women, infants, children, and their families in our community.  

CHS' Healthy Start Coordinator works hand-in-hand with our community by actively identifying and seeking out eligible participants to enroll in the Healthy Start Program. Once in the program, the Healthy Start Coordinator assesses the needs of their participants by conducting risk assessments, developing a plan of care in collaboration with medical providers, provide case work services, conduct home visits, and much more. 

The successful candidate will:
  • Have a bachelor’s degree in social work, social services, health care, education or a related field 

  • Have 2-3 years of experience providing social services to families

  • Have 1 year of experience working with underserved populations 

  • Have a passion for helping improve the lives of women, infants, children and their families

  • Be comfortable conducting field work 

  • Have a deep understanding of the social determinants of health and their impact on health outcomes

  • Be able to communicate effectively and clearly with individuals across a broad range of racial, ethnic and socioeconomic statuses  

  • Have a track record of producing reliable, high quality results with minimal direct oversight

  • Thrive in an environment with tight deadlines and changing priorities where everyone is expected to do a lot with a little

  • Demonstrate alignment with CHS Core Values and a high level of personal accountability

  • Demonstrate compassion and respect for individuals in challenging situations

  • Possess a valid Connecticut driver’s license and a reliable, insured vehicle

  • Demonstrate alignment with the organizational mission

  • Be able to use Microsoft Office products, including Excel
