Since 1970, Community Health Services has existed to provide comprehensive health care services to the residents of Hartford and Windsor. We were made for the community, by the community, to offer a full spectrum of services to include medical, behavioral health care, dental care, a full-service pharmacy and more. Our goal is to offer our patients a medical home; a place where they can come for all of their needs; a place where the clinical staff know them by their first name. Our dedicated staff demonstrate the good we can do on a daily basis when we provide quality care with dignity to each and every individual. We want to help our patients get all of the health care they need -- when they need it -- so they may too live a healthier and happier life.

Community Health Services, Inc. is seeking an experienced full time Infectious Disease Physician to provide care in our growing Infectious Disease clinic.  Our program is evolving from a sole focus on treating individuals with HIV to a broader emphasis on the prevention and treatment of other infectious diseases, including Hep C.  The Infectious Disease Physician will maintain a panel of primary care patients in addition to his or her work in the infectious disease clinic and, with the Director of Infectious Diseases, will act as a subject matter expert for the organization, providing consultative services to other specialties.  Great teamwork produces better outcomes for our patients, so collaboration with the other members of the care team is expected.

The successful candidate will:
  • Be a graduate of an accredited school of medicine and possess Board Certification or be Board-eligible in both Infectious Diseases and Internal Medicine.

  • Possess valid State of Connecticut licensure and have or be eligible to obtain DEA and state controlled substance permit

  • Demonstrate clinical competence related to the diagnosis and treatment of HIV-infected patients and patients with other infectious diseases.

  • Thrive in a collaborative environment, working seamlessly with the Ryan White team, which includes Medical Case Managers, Nurses, and Patient Navigators, in coordinating and co-managing patients.

  • Have a passion for treating individuals with HIV, HCV and other infectious diseases and for educating on prevention as part of our PrEP program

  • Have a strong desire, with the assistance of the Hepatitis C navigator, to develop outreach interventions that identify, reach out to, and retain in care the Hepatitis C population.

  • Be a stickler for timely, thorough and accurate clinical documentation

  • Practice culturally competent care to a diverse patient population with complex needs in a collaborative environment.

  • Have a track record of proven success communicating effectively with patients, peers, and other stakeholders both orally and in writing.

  • Practice with an evidence-based approach and comply with department and organizational quality measures

  • Be comfortable using electronic health record and other computer systems

  • Possess a resourceful, problem-solving attitude and an appropriate level of accountability

  • Be willing to perform other duties as assigned

Candidates with 3+ years’ experience working with medically complex patient populations are strongly preferred.  Candidates who are bilingual in Spanish and English are preferred.
