Community Health Services, Inc. is seeking a qualified and experienced Optometrist to provide care in our Adult Medicine clinic. The Optometrist provides outpatient comprehensive optometric primary care services including preventative and health maintenance services, acute and chronic care, clinical evaluation, diagnosis, and treatment as well as primary optometric care consultation services.

Diagnose diseases and anomalies such as color blindness or glaucoma

Prescribe and fit vision aides such as contact lenses and eyeglasses

Clearly document diagnosis and treatment plan, including referrals, therapeutic prescriptions and long-term medical management, in patients’ charts

Facilitate needed referrals, including social services for patients requiring services not offered at CHS in coordination with referral services representatives and care team nurses.

Review incoming reports (labs, images, etc.) sign-off, and follow up per guidelines.

Adhere to current disease management protocols, and key clinical pathways, and use them to improve clinical performance.

Assist in the review of primary care protocols and patient education materials in collaboration with Clinical Director and/or CMO.

Are a graduate of an accredited school of medicine

Possess valid State of Connecticut licensure and have or will be eligible to obtain DEA

Be willing and able to deliver high quality patient care in an efficient manner

Have 3+ years’ experience

Candidates who are bilingual in Spanish and English are preferred.
