Community Health Services, Inc. is seeking an experienced Asthma Outreach Coordinator for a full time, 40-hour, opportunity in Pediatrics and Adolescent Medicine department. This position is located in Hartford, CT.  Reporting to the Nurse Manager, the Asthma Outreach Coordinator's primary function is to assist providers and nursing staff in providing patient care, specifically coordinating care of the pediatric and adolescent patients with asthma. The Clinical Director of the department is Dr. Zeev Harel, former Brown University Medical School Professor of Pediatrics. The successful candidate will:
  • Professionally coordinate the care of pediatric and adolescent patients with asthma

  • Assist our providers in conducting spirometry and interpreting spirometry

  • Interact with CCMC/Easy Breathing program

  • Interact with Putting on Air program

  • Interact with school nurses regarding implementation of electronic asthma treatment plans

  • Conduct asthma education individually as well as group classes for patients/parents

  • Escort patients to exam room and assist in determining medical problems

  • Take vital signs (blood pressure, weight, pulse, and respiration)

  • Perform medication reconciliation and update allergies

  • Conduct CLIA-waived testing

  • Properly document results in a the EHR

  • Prepare exam rooms and instruments for examination and testing

  • Stock supplies in exam rooms

  • Ensure that lab reports, x-rays and other information are in patients records prior to their visit.

  • Assist providers and nurses with procedures and treatments as directed

  • Attend required meetings, and in-services, and participate in committees/team as requested

  • All other duties and responsibilities as assigned by the immediate manager or practice manager
