Community Health Services, Inc. is seeking an experienced, professional Social Determinant Care Coordinator for a full time opportunity in our Behavioral Health Department. The Social Determinant Care Coordinator is responsible for assisting in coordinating and triaging of daily tasks for our multidisciplinary team to help promote the independence and self-efficacy of our patients. The Social Determinant Care Coordinator, along with their team, will provide outreach, supportive counseling, advocacy, linkage to community supports, and more. 

Essential Functions:

  • Coordinate referrals for basic needs services as necessary with external providers and assist patients with pre-visit planning

  • Compile and provide patient records to external providers as required to complete referrals

  • assist patients in attaining access to care and services

  • Maintain knowledge of health care services and providers in the community and the resources available to support patients' social determinants of health needs

  • Contribute to population health efforts by extracting and analyzing patients utilization information from the electronic health record, data analytics software and other systems.

 

The successful candidate will have:

  • A Bachelor's degree in Social Work or Health and Human Services

  • A CPR certification at the healthcare provider level or be eligible to obtain certification within 90 days or hire

  • Previous experience with referrals or care coordination in a healthcare setting

  • Two (2) years of clinical experience, preferred

  • The ability to maintain focus, objectivity and professionalism in stressful situations

  • Experience working with socioeconomically diverse populations, preferred
